Documentation is required in order to approve accommodations. If you require a form to give to your provider, reach out to the Accessibility Center for a Disability Verification Form. Other sources of disability documentation can include school, college/university, medical provider, disability support agency, etc. Examples of disability documentation include an IEP, 504 Plan, letter of diagnosis from a medical provider and/or neuropsychological evaluation. When submitting documentation, label each item as described by the examples above.
